Protecting Your Personal Data - GDPR (The General Data Protection Regulation)

From 25 May 2018, new data protection regulations mean that I will only be able to contact you if you confirm that you are happy for me to contact you. If I don’t hear from you I will no longer be able to call, email and text you. I’d love to keep in touch, so that you get notifications of appointments, reminders and useful information relating to your treatment. To do so, I need to know that you still want to hear from me. HOW TO GIVE YOUR PERMISSION 1. Read the section below called “How We Use Your Information" 2. Send me an email at lou@amb-associates.com with the following statement: “Dear Lou, I have read and understood the Privacy Policy and I hereby consent to the use of my personal data as described in this statement.  Please contact me by Email, Text, Phone.  Regards [ENTER YOUR FULL NAME]” You may be able to cut & paste this so it should take you less than a minute! WHAT HAPPENS IF YOU DON’T DO ANYTHING From 25 May, I’ll stop calling and sending you any email, or text reminders.  These include messages about consultations, notifications of appointments, reminders and useful information relating to your treatment. HOW I USE YOUR INFORMATION I take your contact details (name, mobile number and/or email address) you provide to inform you of future appointments, or provide you with information which may be beneficial to your ongoing treatment. When you provide your personal contact information, your details are captured, hosted and processed on my mobile telephone. I will also make a file containing the medical history you share with me and I will maintain a record of your treatment. Where there is a legal requirement to hold your data for a defined period of time e.g. for insurance and liability purposes, your information will be held for the duration required by local law. When your personal information is no longer required it will be deleted. Please contact me by email at lou@amb-associates.com if you wish to:

  1. Access, rectify, or erase your details,
  2. Withdraw you consent to me using your details, or
  3. Make a complaint about the handling, processing and security of your personal details.
